Required Documents for Athletic Participation

The following documents and/or forms must be completed and submitted no later than the designated date.
  • 2021/2022 Middle School, JV and Varsity Volleyball: August 26, 2021
  • 2021/2022 Middle School, JV and Varsity Basketball: TBD

STEP 1
SPORTS PHYSICAL
Sports physicals are required for any student to participate in athletics.  You can visit your family physician or other walk-in clinics such as: CVS Minute Clinic, Immediate Care, Med Check, and others.  Neither CVS or Med Check require an appointment.

​Once downloaded, please print and take with you to your physical evaluation appointment.  Once fully completed, please turn this form into the CCS Office by the date designated.  If completed form is not turned in by designated date, your player will not be allowed to participate in team activities, including practices and games.

STEP 2
CODE OF CONDUCT FOR ALL PLAYERS, PARENTS & COACHES
​It is imperative that all players, parents and coaches read, sign and submit (or turn-in) the appropriate code of conduct.  No exceptions.  Without a signature on file, players will not play; coaches will not coach; and parents will be asked not to attend games and practices.

Parents, please take the time to read through the Player Code of Conduct with your child.
